Application and Renewal Information
Paper applications for DRC approval are gone! The application and renewal process for controlled substances is a complete on-line database system. To access the database, please visit: https://protocols.research.colostate.edu/rco/. The approval period for controlled substances is January 1 to December 31 each year; however, you can submit an application at any time during the year. If the application is complete, approval to possess/use/order controlled substances can be obtained within 10-14 days of receipt of the completed application on-line.
Here are a few pointers to help you get started with the on-line system, but if you have any questions, please do not hesitate to call Molly Gutilla, DRC Coordinator at: 491-1553 for further information
- Log onto the system with your standard CSU eID and password. If you have already registered for biosafety approval, you will have access to this database and will now see a “Controlled Substances” panel. If you have not registered, you will be prompted to register. This simply entails entering some basic personal data (e.g., name, email address, telephone number, etc.).
- Once you have registered, click the “Controlled Substance Assurances” link found in the Controlled Substances panel. Fill out the Controlled Substance Assurances statement, indicating at the bottom of the form whether you are a Principal User (PI) or are accessing another faculty’s cabinet. Note: You must complete the Controlled Substance Assurances form to successfully submit your application.
- Any lab personnel who you would like to grant access to your controlled-substance supply will first need to register in the system and sign the Controlled Substance Assurances. Once they have registered, you will be able to add them to your application. To add personnel, click on the “Set Personnel Access to Your Drug Cabinet” link on the Controlled Substances panel. Note: you will need to know this individual’s eName to add them to your group.
- It is also possible to name a delegate who can update your application for you. Once again, this delegate will need to be registered on the system and to have signed the Controlled Substance Assurances. The PI enters a delegate’s eID when registering on the system. If you have already registered, you can edit your profile to include your delegate by clicking on the View or Edit Personal Information tab.
- Once your application is submitted, your request will be automatically submitted to the DRC for review.
- Approval will be sent to you via email.
- For 2008, if you are renewing your approval, you will need to submit your renewal requests by no later than Friday, December 12, 2008.
Drug Transfer Form
Do you have a controlled substance that you would like to transfer to another PI? Are you leaving the university and would like to transfer your controlled substance supply to another CSU PI? Please Use the Transfer Controlled Substance Form.
Please note that the PI who you wish to transfer controlled substances to must also be approved for use of the controlled substance that you wish to transfer to him/her. Please contact Molly Gutilla (491-1553) or Rick Allen (297-1291) with any questions.
Drug Disposal Form
Do you have expired controlled substances that you would like disposed of? Please use the Controlled Substances Disposal Pickup Form and contact Rick Allen to arrange for a pick-up of your controlled substances.

Controlled Substance Log Sheet
Complete record keeping is required at the site of storage for controlled substances. This includes an inventory logbook showing:
- Dates, quantity, and description of each controlled substance received, total quantity ON HAND along with initials of receiving individual.
- Dates, quantity, and description of each controlled substance removed from inventory with description of use, total quantity ON HAND and initials of individual removing from inventory.
- Logbook will show total quantity of each controlled substance ON HAND before removal from inventory and quantity of each controlled substance ON HAND following removal from inventory.
- Logbook to be maintained under security (lock and key) with the controlled substances and made available to member(s) of the DRC for audits.
