Amendments
|
Amendments should be reviewed and approved PRIOR
to the initiation of the change.
Minor amendments can be submitted to the
IACUC Coordinator
via email and should include the following information:
PI name
Protocol number
Nature of change
Justification of change
Major amendments may require the submission of a new A-100. Please contact the IACUC coordinator
for more details.

Most protocols will be freestanding and all content from departmental SOPs should be included in protocol.
